Personal Care Services/ADL's

In our home we offer comprehensive personal care services tailored to each residents needs. Our trained caregivers ensure that residents feel clean, comfortable and well-cared for, always respecting the dignity and privacy for our residents. Our caregivers assist with Activities of Daily Living such as:

  • Bathing/Showering.

  • Grooming, and personal hygiene - Mouth/Denture care.

  • Dressing and undressing.

  • Toileting and incontinence care.

  • Mobility assistance and transferring.

  • Walking.

Medication Management

​Gracious Heart AFH takes medication management very seriously to ensure the health and well-being of our residents. We closely monitor medication schedules, and maintain accurate records to ensure that residents receive their medications safely and on time. Our trained staff assists residents with:

  • Medication reminders

  • Medication administration as prescribed by healthcare professionals

  • Monitoring of medication schedules and refills

Health Monitoring and Care

​Residents in our home benefit from regular health monitoring and personalized care plans. Our dedicated team conducts routine health assessments and monitors daily vital signs to detect any changes or concerns. We:

  • Conduct regular health assessments and monitoring of vital signs daily.

  • Coordinate medical appointments and provide transportation to healthcare facilities/doctors appointments.

  • Collaborate with healthcare providers to ensure continuity of care.

Nutritional Support

​We prioritize nutritious and delicious meals in our home, providing three balanced meals per day along with snacks. For residents who need assistance with feeding or hydration, our compassionate staff is always available to lend a helping hand. We provide:

  • Three nutritious meals per day, plus snacks.

  • Specialized diets and dietary restrictions accommodated (e.g., diabetic diet, low-sodium diet, Puree, thickened liquids)

  • Assistance with feeding and hydration as needed.

Social and Recreational Activities

Keeping our residents active and engaged is a key aspect of our care philosophy here at Gracious Heart AFH. We organize a variety of: ​

  • Planned social activities, games, and exercises to promote socialization and engagement

  • Outdoor outings and community excursions to promote socialization, mental stimulation, and overall well-being.

  • Access to hobbies, crafts, and other recreational activities that cater to their interests and abilities, fostering a sense of community and fulfillment.

  • Music therapy weekly. 

Housekeeping and Laundry Services

​Our home ensures a clean and comfortable living environment for all residents. This includes dusting, mopping floors, and cleaning surfaces to maintain a fresh and inviting atmosphere. We provide regular:

  • Housekeeping services including cleaning of resident rooms and common areas to maintain a tidy and sanitary space. 

  • Laundry and linen services, including washing and changing bed linens to ensure freshness and comfort.

Safety and Security

​The safety and security of our residents are paramount both inside and outside of our home. We:

  • Offer 24-hour supervision and emergency response to address any concerns promptly. 

  • Implement fall prevention measures and conduct regular safety assessments to minimize risks. 

  • Maintain a secure environment to prevent wandering and ensure safety for residents with dementia by having alarms installed on all exits/entrances.

  • Have Safety call button programmed for all rooms.  

Emotional and Psychological Support.

​We understand the importance of emotional well-being for our residents. Our staff offers emotional and psychological support by providing encouragement, companionship, and attentive listening. We foster an environment that promotes independence and autonomy while offering compassionate care and understanding.

End-of-Life Care and Palliative Support:

​During difficult times, our home provides end-of-life care and palliative support to residents and their families. We provide: 

  • Personalized and Compassionate care and support for residents nearing end of life

  • Coordination with hospice services if needed, ensuring that residents receive comfort, dignity, and respect in their final stages of life.  

  • Emotional support for residents and guidance to their families during this difficult time, and helping them navigate this sensitive and challenging time with compassion and empathy. 

Routine Home Doctor

A routine home doctor service offers personalized, patient-centered medical care that prioritizes convenience, comfort, and continuity of care for patients who prefer receiving healthcare services in our home. This comprehensive approach supports patient independence, enhances quality of life, and improves overall health outcomes. Advantages of having a Home Doctor would be:

Convenience - Residents can receive care at the comfort of the home, eliminating the need to travel to the clinic or hospital, saving time and reducing stress for both the resident and the family.

Accessibility - Having a home doctor service improves healthcare, especially for individuals with mobility issues, chronic illnesses.

Comfort and Familiarity - Residents feel more comfortable and secure in the home environment, which can positively impact their emotional well-being and overall quality of life during medical treatment.

Dementia/Alzheimer's and Memory Care

Caring for individuals with Dementia and Alzheimer's disease requires specialized expertise and compassionate support. At Gracious Heart AFH, we are committed to providing exceptional dementia and Alzheimer's care tailored to the unique needs of each resident. Here is how we approach dementia and Alzheimer's care in our AFH:

Person-Centered Approach - We prioritize individualized care plans that focus on each residents preferences, abilities, and cognitive challenges, ensuring personalized support and engagement. 

Memory Care Programs - Our staff undergoes specialized training in dementia care techniques, including validation therapy, reminiscence therapy, and other evidence-based approaches to enhance quality of life and improve our residents sense of well-being. 

Structured Daily Activities - We offer structured and stimulating activities designed to promote cognitive function, memory retention, and social interaction in a safe and supportive environment. 

 

TBI (Traumatic Brain Injury)

At Gracious Heart Adult Family Home, we understand the unique challenges and needs of individuals with Traumatic Brain Injury. Our dedicated team is committed to providing compassionate and personalized care that support recovery, promotes independence, and enhances quality of life. Our caregivers undergo specialized training in TBI care techniques, including cognitive stimulation, behavior management, and adaptive strategies. They are equipped to provide attentive and respectful care while fostering a supportive environment that encourages progress and healing.

Music Therapy

Music Therapy in our home, involves the use of music by a trained therapist to address the physical, emotional, cognitive, and social needs of our residents. It's a therapeutic approach that harnesses the power of music to enhance quality of life and promote well-being. Our music therapist uses various musical activities such as listening to music, singing, playing instruments, and song writing to achieve therapeutic goals.
