The Gracious Heart Adult Family Home team includes well-trained, credentialed, and licensed caregivers to help manage care and chronic conditions such as diabetes, COPD, hypertension, dementia, and health challenges under the watchful supervision of Mark Ngewa, Licensed Provider.

Our caregivers are licensed Home Care Aides (HCA) or Certified Nursing Assistants (CNA). Every staff is also certified in CPR, First Aid, Nurse Delegation, Mental Health and Dementia specialty training, Food Handling, and receives 12 hours of Continuing Education annually. All undergo Criminal Background Checks prior to being hired.

Medication Management

We track, administer, reorder, and manage all aspects of your loved one’s medication regiment, so you never have to worry about reordering or delivering medication.

Meals & Diet

We serve freshly prepared meals and snacks daily. We also accommodate specialized diets such as low salt, low fat, diabetic, and pureed or mechanical soft.

Memory Care & Dementia

We hold dementia and mental health specialty certifications. We can accommodate Alzheimer’s Disease and other cognitive and memory care needs.

Personal Care & ADLs

We provide all personal care such as bathing, personal hygiene, transfers, grooming, feeding assistance, incontinence care, and more.

Prior to moving in, we schedule a comprehensive all-systems’ assessment that addresses the resident's physical and psycho-social needs. From there, a personalized care plan specifically tailored to your physical and psychosocial needs is drafted, with your input.
